You can request to be contacted at a different address, specific phone number, or email address. This is called a “confidential communication request.”

To help protect your information, you can ask SFHP to contact you in a specific way and at a specific location. SFHP will work with your request if we can readily produce your information and notices in the way and location(s) you specify. Your request will be valid until you cancel it or submit a new request.
