If you are a new member, visit your new Primary Care Provider (PCP)* within the first 120 days for an Initial Health Assessment (IHA). The IHA is to help your PCP learn your health care past and needs.

Your PCP may ask you some questions about your health past or may ask you to fill out a health survey. They will also tell you about health classes that may help you. When you call your PCP’s office, please let them know that you are a member of SFHP.

Please take your SFHP ID card to your visit. It is also a good plan to take a list of your meds and questions with you to your visit.

*A Primary Care Provider (PCP) is your main doctor, nurse practitioner, or physician’s assistant.
