If you are a Care Plus (HMO D-SNP) member, you can get an annual wellness visit at $0 cost.

This visit helps you prevent diseases or disabilities.

At your visit, you can:

  • Check your health vitals (like weight and blood pressure)
  • Ask any questions you have
  • Get personal advice
  • Ask about your medicine
  • Plan for future care
  • Get a list of screenings you need (like for breast cancer or colon cancer)
  • Check your brain health
  • Review your medical and family history

During this visit you will fill out a survey, called a “Health Risk Assessment”. This survey will help you and your primary care provider* (PCP) make a personal health plan to help you stay healthy and avoid getting sick. Your PCP is your personal doctor if you need a check-up, want advice about a health problem, or get sick or hurt.
