What Is a Cognitive Screening?

A cognitive screening is a quick test your doctor does to check memory, thinking, and problem-solving. It may include:

  • Simple questions
  • Remembering a few words
  • Talking about your daily activities

It usually takes just a few minutes and is part of your Annual Wellness Visit.

Who Should Get a Cognitive Screening?

If you are an older adult with SFHP Care Plus (HMO D-SNP), you may benefit from a screening if:

  • You are 65 or older
  • You or a loved one notice changes in memory, thinking, or mood
  • You have health conditions like high blood pressure, diabetes, stroke, or depression
  • You have trouble with daily tasks, like paying bills or keeping track of appointments
  • You simply want to understand your brain health

Doctors encourage you to check your thinking when there are any concerns. You know yourself best. Ask for help if something feels different.

Why Screenings Matter

Early checks can:

  • Find problems sooner
  • Connect you with care like food, day programs, or help with medicine
  • Support safe, independent living
  • Give you and your family time to plan
  • Rule out other causes, like stress or medicine side effects
