If you are an SFHP Care Plus (HMO D-SNP) member, here is one simple step you can take today: schedule your Annual Wellness Visit. It is no cost, and it helps you stay on track with your health.

What Is an Annual Wellness Visit?

It is a yearly visit with your primary care provider (PCP). Your PCP is your personal doctor if you need a check-up or want advice about a health problem.

An Annual Wellness Visit is not for when you are sick. It is a time to focus on your overall health and your future.

Why It Matters

  • Check how your body and mind are doing
  • Review your medicines
  • Talk about health screenings to catch problems early
  • Get tips for healthy eating, sleep, and exercise
  • Manage conditions like diabetes or high blood pressure

Even if you feel fine, your health can change over time. This visit helps you stay ahead and feel your best.

Call your PCP today. You can find their number on your SFHP ID card. If you need help or have questions, call SFHP Care Plus Customer Service at 1(415) 539-2273, 1(833) 530-7327 (toll-free), or 711 (TTY), 8:00am–8:00pm.

We are open seven days a week from October–March and closed on Saturdays and Sundays from April–September.
