At San Francisco Health Plan, we reward our members for taking steps to better their health. If you are eligible, you can earn gift cards by going to your health visits.
Why is it important to go to a health visit?
Routine visits to your primary care provider* (PCP) can help you find problems early before they become serious. You can ask questions, get care, and find support to live a long and healthy life.
Your health matters
SFHP’s Health Rewards program helps communities that may have a harder time getting routine check-ups. See below for a list of health visits where you may earn a gift card if you qualify.

First Health Visit

Who can get the reward?
New members within 120 days (4 months) of joining SFHP Medi-Cal. A new member is someone who has not been enrolled in Medi-Cal in the last 12 months.
What do I do to get the reward?
Go to your first health visit with your PCP within 120 days (4 months) of joining SFHP Medi-Cal.
What is the reward?
1 raffle entry for a gift card up to $50.

Prenatal Visit

2019
Who can get the reward?
If you are within your 1st trimester of pregnancy or within 42 days of joining SFHP Medi-Cal.
What do I do to get the reward?
Go to 1 prenatal check-up.
What is the reward?
A gift card up to $50.

Postpartum Visit
Start Date
2019
Who can get the reward?
If you are within 2 to 12 weeks after pregnancy.
What do I do to get the reward?
Go to 1 postpartum visit.
What is the reward?
A gift card up to $50.

Childhood Development Screening
Start Date
2023
Who can get the reward?
A parent or guardian of a child within the first 36 months (3 years) of life.
What do I do to get the reward?
Take your child to their first child developmental screening with a PCP within the first 36 months (3 years) of life.

A gift card up to $50.

Fluoride Varnish Visit
Start date
September 2025
Who can get the reward?
A parent or guardian of a child aged 12 months (1 year) to 47 months (3 years and 11 months).
What do I do to get the reward?
Take your child to get their first and second fluoride varnish treatments with a PCP or dentist in the SFHP Network.
What is the reward?
A gift card up to $50 after each visit.

Colorectal Cancer Screening

Who can get the reward?
Black or African American people aged 45 to 75.
What do I do to get the reward?
Go to your first colorectal cancer screening with a PCP.
What is the reward?
A gift card up to $50.

Well-Child Visit (First 15 months)
Start Date
September 2025
Who can get the reward?
A parent or guardian of a child within the first 15 months (1 year and 3 months) of life.
What do I do to get the reward?
Take your child to 6 or more well-child visits with a PCP within the first 15 months (1 year and 3 months) of life.
What is the reward?
A gift card up to $50.

Well-Child Visit (Ages 3-11)

September 2025
Who can get the reward?
A parent or guardian of a child ages 3 to 11 years old. Must be Black/African American, Native Hawaiian, Pacific Islander, American Indian, or Alaska Native.
What do I do to get the reward?
Take your child to an in-person well-child visit with their PCP or OB-GYN before the end of the year.
What is the reward?
A gift card up to $50.

Well-Child Visit (Ages 12-21)

Who can get the reward?
People ages 12 to 21 years old. Must be Black/African American, Native Hawaiian, Pacific Islander, American Indian, or Alaska Native.
What do I do to get the reward?
Go to 1 in-person well-care visit with your PCP or OB-GYN before the end of the year..
What is the reward?
A gift card up to $50.
You do not need to fill out any forms to get a gift card! Rewards will be automatically mailed to you after 8 weeks following your health visit(s). If you have not received your gift card after 8 weeks, please call SFHP Customer Service at 1(415) 547-7800, 1(800) 288-5555 (toll-free), or 711 (TTY), Monday–Friday, 8:30am–5:30pm.
If you have moved recently, please call SFHP Customer Service to update your current mailing address to make sure you get your reward.
You may not use gift cards to buy firearms, alcohol, or tobacco.
*Your primary care provider (PCP) is your main doctor or healthcare provider who helps you stay healthy and handles your care.
