Medi-Cal services offer you extra help at no cost

Community Supports (CS) are special care services for SFHP Medi-Cal members. CS services can help you find housing and pay for home updates.

The new programs that start January 1, 2024, include:

  • Housing Deposits cover one-time move-in costs if you found a home through SFHP’s Housing Navigation Services.
  • Housing Tenancy and Sustaining Services help you to stay safe and keep your housing.
  • Environmental Accessibility Adaptations (or Home Changes) are changes to your home that will help you live safely and on your own. This can be:
    • Ramps
    • Stairlifts
    • Making doorways wider for wheelchair use
    • Making bathrooms and showers wheelchair accessible
  • Nursing Facility Transition helps when you live in a nursing facility and want to safely move and need support.

*Your primary care provider (PCP) is your main doctor, physician assistant, or nurse practitioner.

  • Interpreter Services: You can get an interpreter in-person or by phone for your health visits. When you make an appointment, ask for an interpreter at the same time.
  • Need a Ride? SFHP can help you get transportation to any medical appointment covered by Medi-Cal. Ask your provider or call Customer Service.
