  • $0 for all covered prescription drugs
  • $90 per month for over-the-counter items (OTC) such as vitamins, aspirin, first aid, etc.
  • $50,000 per year for emergency and urgent care while you are traveling out of the USA

  • Complete your Health Risk Assessment survey each year

Vision

  • $0 annual eye exam
  • $300 for glasses or contacts
Dental

  • $0 crowns, rebasing dentures, bridges
  • $0 exam beyond Medi-Cal coverage
Acupuncture

  • $0 for 24 routine acupuncture visits in addition to Medicare/Medi-Cal
Chiropractor

  • $0 for 12 routine chiropractic visits in addition to Medicare/Medi-Cal
Hearing Aids

  • $2,000 allowance for prescribed hearing aids, $0 hearing exams

*These benefits are part of the Special Supplemental Benefits for the Chronically Ill (SSBCI). We will notify you if you meet the requirements (must have chronic condition such as diabetes, hypertension, must be at high risk for hospitalization/complex care, and must require care coordination.

SFHP Care Plus (HMO D-SNP) is an HMO plan with a Medicare and Medi-Cal contract. Enrollment in SFHP Care Plus depends on contract renewal.
