Quality Improvement and Health Equity Program

San Francisco Health Plan’s Quality Improvement program was recently renamed to the Quality Improvement and Health Equity Transformation (QIHET) Program. This program establishes methods for monitoring, evaluating and improving the quality of care and services provided to SFHP’s members. The QIHET Program Description and Plan outlines SFHP’s infrastructure to support quality improvement and health equity as well as identifies annual goals. SFHP evaluates progress towards our goals at the end of each year through the creation of a QIHET Program Evaluation. For more information please email the Quality Improvement team.
