Incentives Available to Members

  • Contact the Health Education Department to request incentive materials (forms and prepaid envelopes), or for a presentation to your staff about SFHP’s incentive programs.
  • Fax incentive forms directly back to SFHP at 1(415) 615-4547.
  • Members should call SFHP Customer Service at 1(800) 288-5555 if they have not received a response in 4-6 weeks of submission.

Member Incentive Card

For Medi-Cal ages 0 through 15 months.

Children who receive 6 well-child visits during the first 15 months of life.

Members receive mailed information once each year.

A chronic condition is a health issue that lasts 1 year or more. It needs ongoing medical help and may limit you from doing daily activities. Chronic conditions can often be controlled. Your care team can help you have a healthy life with your chronic health condition.

Only for members with asthma, diabetes, or high blood pressure (hypertension).

The member or the provider do not have to fill out any forms to get the gift card.

The gift card will be automatically mailed to the member after 5 weeks following their health visit(s). If they have not received their gift card after 5 weeks, please advise them to call SFHP Customer Service at 1(415) 547-7800 or 1(800) 288-5555 (toll-free) or 1(415) 547-7830 (TTY). Monday through Friday 8:30am to 5:30pm.
