Stay Healthy, Feel Your Best

SFHP is here to help you get the best care possible, so you can lead an active and healthy life – however that looks for you. Getting timely care in the language you speak can help you stay on top of your health, and avoid or manage more serious health issues.

Preventive Care Services

SFHP covers many preventive services at no cost to you. These services can help you maintain your best health and find any serious health problems before they get worse. Covered services include:

Routine check-ups with your primary care provider (PCP) can help you stay healthy and talk about any health concerns you have.

Screenings can find health problems early, so you can get the treatment you need. You can get screened for diabetes, high blood pressure, sexually transmitted infections (STIs), and more. Learn how you can earn gift cards for certain health screenings.

Vaccines (shots) can protect you from serious diseases like the flu, measles, and tetanus (or “lockjaw”).

Well-care visits help make sure that kids are growing and developing well. Your child can get the health screenings, shots, and support they need at a well-care visit. Find out more about care for kids up to age 21.


Schedule a Check-up with Your PCP

Within 4 months (120 days) of joining SFHP, you must complete a check-up with your primary care provider (PCP). Don't wait until you get sick. Scheduling a visit now can help you:

  • Get to know your PCP
  • Share more about your health needs
  • Get care more quickly in the future

During your check-up, your PCP can help you get any screenings, shots, or tests you need. Ask your PCP what shots and screenings are right for you.

If you are under age 18, you can get care for sensitive health issues without your parent or guardian’s permission. Topics may include pregnancy, STI/HIV prevention, treatment for substance use, and more.

Support Groups »

It can help to talk about your feelings and experiences in a safe place. There are many support groups and classes where you can talk with other people about mental health, substance use, grief, and more. Find a group near you.


How to Get Care

Getting care with SFHP is easy. Call your PCP if you need a check-up, want advice for a health problem, or you get sick or hurt. Your PCP can also help you get specialty care if you need it.

Your PCP’s phone number is on the back of your SFHP Member ID card. Or, you can find a PCP near you in the Provider Directory. If you need help finding your PCP’s number, please call SFHP Customer Service at 1(888) 883-7347 (711). SFHP Customer Service is available Monday through Friday, from 8:30am – 5:30pm.
