San Francisco Health Plan contracts with ten medical groups and their affiliated hospitals for clinical services. Individual physicians, other health care providers, and clinics participate in our network through one of these groups.
Your Primacy Care Provider (PCP)
The PCP or clinic you choose will oversee all of your health care needs. If you need specialty care, your PCP or clinic will help you find other health care providers, called specialists, and will refer you to them when needed.

How To See a Specialist
You can visit a specialist if you need expert care for a specific type of problem. Specialists are surgeons, heart doctors, allergy doctors, skin doctors, and other doctors who work in one area of health care. Most specialists will ask you for a referral from your primary care provider (PCP) before you make an appointment.

All American Medical Group (AAMG) is a medical group with over 150 physicians throughout San Francisco, including primary care physicians (PCPs) and specialists. Many of AAMG’s physicians speak Chinese dialects, as well as other Asian languages. Community Clinic Network is made up of independent non-profit community clinics across San Francisco. They provide quality health care in culturally, linguistically, and population sensitive, community-based settings. Comprehensive primary, preventive, and ambulatory care services are provided by a team of doctors, mid-level clinicians, nurses, dentists, and health practitioners. Many clinics offer “one stop shopping” including mental health services, dental and vision care, health promotion, and disease prevention education. Specialty services and emergency care are provided by Zuckerberg San Francisco General Hospital and Trauma Center and skilled nursing and rehabilitation is available at Laguna Honda Hospital.

Compliance and Delegation Oversight Summary of Delegation Model
We set goals to give you easy access to our providers. We sign up doctors, clinics and hospitals into groups, called networks. This helps us with quality and costs. We call this groups of providers, “delegates.” We work with these delegates to make our service to you better. They have to follow the same rules that we do.
When you choose a primary care provider (PCP), you are also choosing a delegate. This includes all the doctors and hospitals within their network. We have them take care of referrals and prior authorizations for you. They also review and approve providers to join them. This is called credentialing. We also have them pay providers.
We make sure they take care of you. We do this by working with them and checking to see they are following the rules. We check that they give our members great service. We review them each year to make sure they follow laws and their contract with us.
