Community Clinic Network is a group of independent non-profit clinics in San Francisco. They offer quality health care that respects different cultures and languages in community-based settings. A team of doctors, nurses, dentists, and other health workers provides primary care, checkups, and more. Many clinics offer “one-stop shopping.” This includes mental health care, dental and vision care, health education, and help preventing diseases. For specialty and emergency care, patients go to Zuckerberg San Francisco General Hospital and Trauma Center. Laguna Honda Hospital provides skilled nursing and rehabilitation.

Compliance and Delegation Oversight Summary of Delegation Model
We set goals to give you easy access to our providers. We sign up doctors, clinics, and hospitals into groups called networks. This helps us with quality and costs. We call these groups of providers “delegates.” We work with these delegates to make our service better for you. They have to follow the same rules that we do.
When you choose a primary care provider (PCP), you are also choosing a delegate. This includes all the doctors and hospitals within their network. We have them take care of referrals and prior authorizations for you. They also review and approve providers to join them. This is called credentialing. We also have them pay providers.
We make sure they take care of you and provide great service. We do this by working with them and reviewing them each year to make sure they follow laws and their contract with us.
