San Francisco Health Plan contracts with nine medical groups and their affiliated hospitals for clinical services. Individual physicians, other health care providers, and clinics participate in our network through one of these groups.

Your Primacy Care Provider (PCP)

The PCP or clinic you choose will oversee all of your health care needs. If you need specialty care, your PCP or clinic will help you find other health care providers, called specialists, and will refer you to them when needed.

Chinese Community Health Care Association is a medical group with over 150 doctors from the Chinese Hospital medical staff. Their doctors speak most Chinese dialects as well as many other Asian languages and are located throughout San Francisco. Hospitalization is at the bilingual Chinese Hospital that has 24-hour emergency services. A walk in Customer Service office is conveniently located at the Chinese Hospital Office Building.

We are committed in delivering the highest quality, culturally sensitive, bilingual managed health care to our community. The network hospital for Jade Health Care is Chinese Hospital. If your Jade Health Care specialist works with another hospital, such as California Pacific Medical Center or University of California, San Francisco, you might get needed hospital care at the other hospital. Kaiser Permanente attracts and selects top quality health care providers. Members have the ease of receiving complete health care services under one roof. These services include preventive care, specialty care, lab, pharmacy, X-rays, extended and weekend hours, advice nurses 24 hours a day, and on site member service staff to assist members with questions and requests.
