Transitional Care Services

SFHP members may be eligible for Transitional Care Services (TCS). TCS is for members who transfer from one setting or level of care to another and need help after they are discharged from the hospital, institution, other acute care facility, and skilled nursing facility to home or community-based setting, Community Supports placements (including Sobering Centers, Recuperative Care, and Short-Term Post Hospitalization), post-acute care facilities, or long-term care (LTC) settings.

TCS can help you:

  • Review your discharge instructions and help you get answers from the right people.
  • Schedule your care visits after you are discharged.
  • Find a licensed clinician who can help you with medications after you are discharged.
  • Provide community referrals.
  • Get a ride to your medical visits.
  • Manage your providers.
  • Find out what other Care Management programs you may be able to get.

Transitional Care Services for Pregnant Members

This program supports members during pregnancy (“prenatal”) and after birth (“postpartum”).

This program can help you:

  • Start the birth support checklist with the help of your care team
  • Get support for reproductive life planning
  • Connect to primary care
  • Schedule needed health visits before and after birth
  • Connect to support like doula, housing, behavioral health, infant health insurance
  • Get many other resources!

Your care team will support you every step of the way from the beginning of your pregnancy through 12 months after birth.

Birthing Supports Coordination Checklist

Medical Supports

  • Postpartum visit
    • Validated Behavioral Health Screenings
    • IPV (Intimate Partner Violence) Screening
    • Reproductive Life Planning
  • Child (“pediatric”) visits scheduled through 2-month well-child visit
  • Any follow-up visits suggested by a care provider
  • Primary Care Provider visit (if no visit scheduled within the past 1 year)
