Incentives Available to Members
- Contact the the Health Education Department to request incentive materials (forms and prepaid envelopes), or for a presentation to your staff about SFHP’s incentive programs.
- Fax incentive forms directly back to SFHP at 1(415) 615-4547.
- Members should call SFHP Customer Service at 1(800) 288-5555 if they have not received a response in 4-6 weeks of submission.

SFHP no longer offers an Asthma member incentive program. Also, the Childhood Immunization Member Incentive Program will end on June 30 2020. If your patient received either of these forms in the mail, please advise them to return it to SFHP via mail or your office may fax it to us directly at 1(415) 615-4547. SFHP will process the completed incentive form for these programs as usual.
The blank asthma and childhood immunization incentive forms are no longer available on our member-facing website or our provider resource page. Please continue to give recommendations to your patients about immunizations and asthma. You may also direct patients to the SFHP website for health education resources at https://www.sfhp.org/health-wellness/health-education-library/.
For Medi-Cal ages 0 through 15 months.
Children who receive 5 well-child visits during the first 15 months of life.
Members receive mailed information once each year.
For Medi-Cal pregnant members.
Pregnant members who receive a prenatal checkup within the required time frame (42 days for new members and first trimester for existing members).
Members can call SFHP to request a submission form.
For Medi-Cal members who have given birth.
Members who receive a postpartum health visit between three and eight weeks after delivery.

The Diabetes Incentive Program has ended. If you received this form in the mail in Spring of 2019, please return it to SFHP by mail or you may have your provider fax it to us. SFHP will process your filled in incentive form for this program as usual.
The Hypertension Incentive Program has ended. If you received this form in the mail in Spring of 2019, please return it to SFHP by mail or you may have your provider fax it to us. SFHP will process your filled in incentive form for this program as usual.
