San Francisco Health Plan issues each Member an ID Card. When you get your Member ID Card, check all information to make sure it is correct. You may have a new ID Card mailed to you at no charge if:
1. Any information is incorrect
2. Your card is lost of stolen
3. You change your primary care provider
4. Your address or other information changes
To request a new Member ID Card, call our Customer Service Team at 1(800) 288-5555, Monday-Friday, 8:30am – 5:30pm.
