We do not collect your personal information on this Website unless you voluntarily provide it to us, as further described below. By using SFHP’s Website, you are agreeing to the Website Terms and Conditions and the practices described in our Privacy Policy. When you access our Website, we do not collect personal information about you unless you voluntarily submit such information. Portions of our Website may allow you to provide us with Personal Information in order to deliver requested materials, products or services to you, submit a grievance, respond to your questions, or enable to you to update information. You represent that Personal Information submitted in the registration process is true and accurate and you agree that you are solely responsible for the security of your password and account.
